• 특징
  • 금주 후 수 시간 후 부터 초기증상, 1~2일(24~48시간 후)에 peak
  • Benzodiazepine : immediate onset
    • lorazepam(Ativan), diazepam(Valium)
  • Administer vitamin B₁ (Thiamine)
    • Wernicke’s encephalopathy.
• Complications
  • Vitamin B deficiency → peripheral neuropathies, Wernike’s encephalopathy(Confusion, ataxia, abnormal eye movements, altered mental state)
  • Thiamine deficiency → Korsakoff’s syndrome(X reversible, confusion to death, ataxia, vision change, memory loss)
  • Brain damage → severe memory problems → 알코올성 기억상실(Alcohol-induced persistent amnesic disorder)
  • Hepatitis; LC, Esophagitis and gastritis, Pancreatitis
  • Anemias, Immune system dysfunctions
  • Brain damage, peripheral neuropathy
• 초기 증상
  • Anorexia(N/V)
  • Anxiety
  • Easily startled, Hyper alertness, Irritability, shaking
  • Hypertension, insomnia, agitation, tachycardia
  • Jerky movements tremors
  • Hallucinations, illusions, delusions, or vivid nightmares
  • Seizures
• 악화 → Delirium Tremens(DT)
  • Fever (100℉[37.8℃]] to 103℉[39.4]℃)
  • Diaphoresis
  • Disorientation, 혼란, 의식 저하
  • Hallucinations, delusions 악화
  • Tachycardia , HTN

  • 평가 항목 : 각 항목은 0~7점 척도(일부 항목은 0~4점)로 평가
    1. 오심 및 구토 (Nausea and Vomiting)
    2. 손 떨림 (Tremor)
    3. 불안 (Anxiety)
    4. 발한 (Paroxysmal Sweats)
    5. 착란 (Clouding of Sensorium, 의식 혼돈)
    6. 환각 (Hallucinations) - 시각(Visual), 청각(Auditory), 촉각(Tactile)
    7. 두통 (Headache or Fullness in Head)
    8. 운동성 초조 (Agitation)
    9. 지남력 장애 (Orientation and Clouding of Sensorium)
    10. 시각 및 청각 장애 (Visual and Auditory Disturbances)
  • 점수 해석 및 관리
    • 0~9점: 경미한 금단 증상 → 특별한 치료 필요 없음
    • 10~19점: 중등도 금단 증상 → 약물 치료 고려 (예: 벤조디아제핀)
    • 20점 이상: 중증 금단 → 입원 치료 필요, DT(섬망진전) 위험 있음
